Exploring New Medi-Cal Services in San Diego County
The State of California is rolling out exciting new services under Medi-Cal that aim to assist some of the most vulnerable community members in San Diego County. Enhanced Care Management and Community Supports are designed to provide tailored care for individuals facing challenges such as homelessness, substance use disorders, mental health issues, and former foster youth. These innovative services are available at no cost for eligible members enrolled in one of four Medi-Cal Health Plans: Blue Shield Promise, Community Health Group, Kaiser Permanente, or Molina Healthcare.
What Are Enhanced Care Management and Community Supports?
Enhanced Care Management offers a dedicated care manager for Medi-Cal members. This professional coordinates members' healthcare needs and connects them with vital services, which can include arranging transportation to appointments, explaining medication and insurance, and making necessary health appointments. It's a personalized approach to help members navigate the complexities of healthcare.
On the other hand, Community Supports are short-term services that assist individuals in achieving specific goals. Eligible members can benefit from services like:
- Housing Aid: Assistance with developing housing plans, conducting housing searches, paying security deposits, and helping cover initial utility bills.
- Recovery Services: Options include safe post-hospital care environments, sobering centers, and provisions for healthy meals.
